► PLAN 1: “PAY-WITH-INSURANCE” FOR SERVICES
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
♦ Do I have mental health insurance benefits?
♦ What is my deductible and has it been met?
♦ How many sessions per year does my health insurance cover?
♦ What is the coverage amount per therapy session?
♦ Is approval required from my primary care physician?
► PLAN 2: “PAY-AS-YOU-GO” FEES FOR SERVICES
♦ $175 Initial 60-min. Diagnostic Evaluation Session
♦ $100 Subsequent 45-min. Individual Psychotherapy Session
♦ $125 Subsequent 45-min Family/Couples Psychotherapy Session
♦ $150 One Hour of Psychological Assessment/Interpretation ♦ A full fee schedule is available upon request.
Out-of-Session Services (Non-Emergency)*
Out-of session services are billed based on a fee of $125 per hour. They include:
♦ phone calls longer than 10 minutes to patients or on their behalf;
♦ non-emergency e-mail consultations requiring more than 10 minutes;
♦ letters requested from me; etc.
One-Time Fee Alternative for Out-of-Session (Non-Emergency) Services*
♦ Individual fees for out-of-session, non-emergency calls, letters, professional consultations
requiring more than 10 minutes are waived for patients who choose to pay a $125 annual fee.**
► PLAN 3: PAY-IN-ADVANCE FOR SERVICES

♦ If you expect to attend therapy on an ongoing basis, you may benefit from a courtesy discount by paying for services in advance, on a monthly basis. Please contact us to discuss your needs and options.
CANCELLATION POLICY: 2 BUSINESS DAY + IN PERSON/ BY PHONE
Your appointment time is reserved exclusively for you. If you discover that you won’t be able to attend your appointment, to avoid a charge for the session, I require notification in person or by phone within at least two(2) full business day (48 hours). For example, to cancel a 9 a.m. appointment on Monday morning, I will need to hear from you on the previous Thursday by 9 a.m., at the latest.
I am not typically able to fill appointment times with less than one business day’s notice. For this reason, you will be billed the full session fee for a missed or late-cancelled appointment.
FORMS OF PAYMENT ACCEPTED
Payment is due in full on the date of service.
